It is our policy that every client and caregiver is entitled to be treated with dignity and respect, without discrimination on the grounds of race, color, religion, national origin, military status, age, gender, or disability. The relationship between clients and caregivers shall be characterized by mutual respect, including respect for personal property, and adherence to ethical standards of conduct at all times.
As a client, you are entitled to the following rights:
You shall receive comprehensive information regarding the services provided by Sohm Family Care, including a detailed explanation of the care to be rendered, the qualifications of the caregiving personnel, and the proposed schedule of visits.
You shall be informed of the identity and contact information of the individual responsible for supervising the provision of care, including methods for contacting this individual outside of standard business hours.
You have the right to lodge complaints or grievances without the fear of retaliation and to have such concerns addressed promptly and effectively.
You are entitled to participate actively in the formulation and ongoing review of your Plan of Care, to be apprised of your responsibilities within the care process, and to be notified in advance of any modifications to your Plan of Care.
You have the right to access the state registry of licensed agencies and to select any licensed entity to provide the requisite care, where applicable.
You are assured of courteous and respectful treatment, including the safeguarding of your personal property, privacy, and freedom from abuse, neglect, exploitation, and discrimination.
You shall have access to and confidential management of your client records and Protected Health Information, in accordance with state laws and HIPAA regulations.
You shall receive a detailed billing statement that itemizes services rendered, including the date and associated charges.
You shall be informed about Advance Directives and the responsibility of Sohm Family Care to convey such directives to the appropriate medical personnel when available.
You reserve the right to decline services and to be informed of the potential repercussions of such a decision.

Should you perceive any failure on the part of our staff to adhere to our policies, or if you believe your rights have been compromised, we encourage you to submit a complaint.
